Valid Class 5 driver's license

Must have your own vehicle

Good standing with BCCNP

Up to date First Aid, CPR, WHIMS and Immunizations

Monitor patient conditions and chart/report on patient progress

The Roles:

We are looking for enthusiastic and motivated LPN's to join our growing healthcare team servicing Abbotsford, Mission, and Aldergrove. A typical work day will involve visiting patients in their own homes to provide witnessed medication administration, injection of SC/IM meds, monitoring vitals, providing education to patients on how to self monitor conditions such as diabetes, monitoring the progress of current morbidities (i.e. cardiovascular health, mental health, pain management).


The LPNs work in conjunction with an LPN supervisor in developing and maintaining optimized care plans for patients in the community setting.

Qualifications and Competencies and Requirements:


Graduation from a recognized Resident Care Aide (RCA) program.

Current registration with the BC Care Aide Registry is required.

Must have own reliable motor vehicle

BC driver’s license

Physically able to lift/turn and lift 23 kg safely.


Documents/Training:


Criminal record check (vulnerable persons); MUST be within six months and state “Casa Health/HealthLink Outreach.”

Basic first aid training (including CPR).

Casa Health / HealthLink Outreach fit to work form (to be completed by physician)

Foodsafe Level 1 certificate, or certificate from a course recognized as equivalent to Foodsafe.

The Role:

The Community Registered Care Aide (C-RCA) acts as an integral member of the Casa Health/HealthLink Outreach care team and assisting in delivering patient care while maintaining a person-centered approach to all care provided in each patient’s home. They will act as an integral member of the patient(s) care team to support each individual to maintain their independence, dignity and holistic well-being.


Key Responsibilities:


Following established care plan(s), the Casa Health/HLO C-RCA provides care services to patients, including, but not limited to:


Administering pre-dispensed medications to patients per scheduled assignment/route list(s), provided by Casa Health/HLO, as training/certification permits, only.

Working in close collaboration with the Casa Health/HLO nursing and pharmacy team(s) to provide safe, competent, and appropriate support to chronically ill patients.

Completing basic vital signs monitoring and tracking, utilizing appropriate automated equipment and assigned documentation.

Reporting emergencies and care issues per the “What, When, How” sheet to nurse leadership and/or pharmacy, immediately.

Completing Casa Health/HLO medication and C-RCA training and education per schedule. Participates in ongoing training (annual education calendar, etc.)

Striving to build rapport with and support/encourage each individual patient to be at ease with self/team, thereby allowing for more effective care partnerships to be developed/maintained.

Observing and monitoring client/resident progress, symptoms and behavioral changes and reporting/documenting significant observations in each patient’s “PN/C-RCA PROGRESS NOTES.”

Maintaining a clean and safe environment for clients and staff.

Maintaining equipment and supplies per “C-RCA Bag Sheet”.

Performing other related duties per delegated or assigned tasks ONLY.

Another Lovely Review

When I first started nursing school, I was adamant in knowing I did not want to work as a community nurse going into patients homes. I did not think I would enjoy it and did not want to be alone with patients because I felt like I would not get any support and I would be all alone every time but here I am today, thrilling However, when I was working at a care home one of my co-workers referred me to my current boss and suggested I go for the interview.


I have grown to love community nursing because it gives me more time to spend and talk with my patients one on one. The Pharmacy staff and my boss at Pharmacity Drugstore are super supportive and care beyond their line of duty. They go out of their way to make sure I am doing okay and it really shows.


The people I work with have become family to me, whether that is the Pharmacy team, my boss or my patients. It is hard not to build relationships with patients when I am with them all the time. I definitely have my favorite patients that I go out of my way for. For example: on my favorite patients' birthdays I usually bring them a small piece of cake and card, because I know they make my day a little better and I have been told I make their day on a daily basis. Sometimes my boss comes out with me for certain patients' birthdays. I do not know anywhere else where the BOSS comes out with the nurse to take time out of their busy day and their OWN job to see a patient just because it is their birthday.


This place is definitely something REALLY special and I will miss it when I leave. It makes me sad to leave my favorite patients because I know how much I have impacted them.

“Aw I don’t like that, you are my best girl. You are my favorite”

“You are one of my girls now”

“You make my day”

“You have such a calming presence around me. I knew it as soon as I saw you”

“You aren’t going anywhere until I die. You can’t leave me. I need you. Why can’t you come everyday”

“Stay and talk with me. What do you wanna talk about? Let’s play a game”

“I better see you at least once a week. You make me so happy”


I know I will dearly miss the one on one and time spent with each and everyone of them because of the way they have impacted my HEART, and my life.


It also saddens me to leave my boss because I know she relies on me for everything. She deeply cares and she is generally a fantastic person and someone you want in your life and by your side.


If you choose to have this Pharmacy care for your loved one, you will not be disappointed and you will not have to worry if they are being well cared for. The family members of our patients’ value the communication about their loved ones health status between the nurse, and pharmacist. To know that there is continued communication going on. It gives them a sense of ease. If I had a loved one and they needed help with medication administration I would choose this pharmacy without thinking.


I hope my testimony paints a picture of my experience with this company and what it has done for me and, hopefully, you took something away from it.
